West Wales Care Partnership Board

Carer Strategy Survey

The Welsh Government defines carers as: “anyone of any age, who provides unpaid care and support to a relative, friend or neighbour who is disabled, physically or mentally ill, or affected by substance misuse”.

The Health and Social Care organisations in west Wales are developing a regional strategy for unpaid carers which will be endorsed by the West Wales Regional Partnership Board. This survey is based on the three key themes identified by the Welsh Government:

• Identification of carers
• Supporting life alongside caring
• Providing information, advice and assistance (IAA)

The survey is intended for unpaid carers and organisations that support unpaid carers. Please circulate the link to this survey through your networks.

Arolwg Strategaeth Gofalwyr

Diffiniad Llywodraeth Cymru o ofalwyr yw: “unrhyw un, o unrhyw oed, sy’n darparu gofal a chefnogaeth yn ddi-dâl i berthynas, cyfaill neu gymydog sy’n anabl, sydd â salwch corfforol neu feddyliol, neu sydd wedi’i effeithio gan gamddefnyddio sylweddau”.

Mae sefydliadau Iechyd a Gofal Cymdeithasol yng ngorllewin Cymru yn datblygu strategaeth ranbarthol ar gyfer gofalwyr di-dâl a fydd yn cael ei chymeradwyo gan Fwrdd Partneriaeth Ranbarthol Gorllewin Cymru. Mae’r arolwg hwn wedi’i seilio ar y tair thema allweddol a nodwyd gan Lywodraeth Cymru:

• Adnabod gofalwyr
• Helpu i fyw yn ogystal â gofalu
• Darparu gwybodaeth, cyngor a chymorth

Mae’r arolwg wedi’i anelu at ofalwyr di-dâl a sefydliadau sy’n cefnogi gofalwyr di-dâl. Cylchredwch y ddolen hon ymhlith eich cysylltiadau, plîs.
