Health Passports

A Health Passport is a booklet that a person with a learning disability can take with them when attending health appointments, including hospital stays. It contains important information on how to communicate and support the individual. People who don’t know an individual well, such as a doctor or nurse, can read the Health Passport and make a connection with that individual.
